If you visit our site to register for a Work Incentive Seminar Event (WISE), we collect and store the following information to conduct our WISE webinars:

  • Name
  • Mailing and E-mail Address
  • Phone Number
  • Registrant Type
  • Event Accommodations

You also may decide to send us personally-identifiable information, for example your mailing address, in an electronic mail message requesting that information be mailed to you. Information collected in this manner is used solely for responding to requests for information or records. We may forward your e-mail to other government employees who are better able to respond to you; however, we will not share any of your personal information with any other organization or individual. Should you wish to file a request under the Freedom of Information Act (FOIA), instructions are provided at http://www.ssa.gov/foia/index.htm.

Section 205(a) of the Social Security Act, as amended, authorizes us to collect the information requested on this form.  Social Security will only use your contact information to register you for a WISE webinar and to provide you with related Ticket to Work program information that may benefit you. The Privacy Act (5 U.S.C. § 552a(b)) permits us to disclose the information you provide on this form in accordance with approved routine uses. Giving us this information is voluntary; however, if you do not complete the required fields we cannot provide you with these services.  Additional information regarding this form, routine uses of information, and other Social Security programs, is available on our Internet website, www.socialsecurity.gov, or at your local Social Security office.

Finally, the Privacy Act of 1974, as amended (5 U.S.C. 552a), requires us to notify you that the Privacy Act protects the information you are providing to us. The Privacy Act also requires we notify you that, in rare instances, we may have to disclose this registration information to the Department of Justice or a court in connection with Federal litigation.

If you sign up for email updates from the Ticket to Work program, we collect and store the following information to provide you with periodic email updates:

  • Name
  • Email Address
  • Phone number on which you received a call from Social Security about the Ticket to Work program
  • Mobile phone number if opting in for text messages

If you sign up for email updates we WILL send you information about the Ticket to Work program and other Social Security benefit or program information that might be of value to you. We WILL NOT provide your email address to other organizations or individuals for the purposes of commercial solicitation or any solicitation unrelated to Social Security. Social Security will only use your email address to keep you informed of Social Security topics and programs that can benefit you. We will disclose information collected and maintained in this system only to Social Security employees and contractors who will use it to send you official Social Security information that could be helpful and informative to you.

You may unsubscribe from Ticket to Work program updates at any time by emailing the word UNSUBSCRIBE in the body of an email addressed from the email account from which you signed up to support@chooseworkttw.net.

Text messages will require an additional confirmation by you that you want to receive them, and instructions on how to stop them will be provided when you confirm your subscription. Standard text messaging rates will apply.
